Gynecomastia, or enlarged male breast tissue, affects more than half of men at some stage of life. It can be caused by hormonal changes, genetics, certain medications, or other factors. For many, no amount of diet or exercise improves the condition, leaving surgery as the most effective solution.
Seeking treatment is a personal decision—and one that deserves respect, understanding, and the highest level of surgical expertise.

Gynecomastia FAQs
What exactly is gynecomastia?
Gynecomastia is a medical condition characterized by the enlargement of breast tissue in males due to an imbalance of hormones, particularly estrogen and testosterone. Unlike pseudogynecomastia (which is caused by excess fat), true gynecomastia involves actual glandular breast tissue that cannot be reduced through diet and exercise alone. This condition affects up to 70% of men aged 50-69 and can occur at any age, from puberty through adulthood.
How do I know if I have gynecomastia or just excess fat?
True gynecomastia involves firm, rubbery tissue directly behind the nipple that can be felt during a physical examination. This glandular tissue is typically tender and forms a disc-like shape under the nipple-areolar complex. Pseudogynecomastia, on the other hand, consists of soft fatty tissue that’s more evenly distributed across the chest. During your consultation, Dr. Jaffer will perform a thorough examination to determine which type of tissue enlargement you’re experiencing, as this affects your treatment options.
What causes gynecomastia to develop?
Gynecomastia can result from various factors including natural hormonal changes during puberty or aging, certain medications (such as anti-androgens, antibiotics, or heart medications), medical conditions affecting hormone levels, liver or kidney disease, hyperthyroidism, or the use of substances like marijuana or anabolic steroids. In many cases, the exact cause remains unknown (idiopathic gynecomastia). Understanding the underlying cause is important for treatment planning and preventing recurrence.
Can gynecomastia resolve on its own?
In adolescents, gynecomastia that develops during puberty often resolves naturally as hormone levels stabilize, typically within 6 months to 2 years. However, gynecomastia that persists beyond age 17 or develops in adulthood rarely resolves on its own and typically requires surgical intervention for correction. If you’ve had enlarged breast tissue for more than two years, it’s unlikely to improve without treatment.
What surgical techniques are used for male breast reduction?
Dr. Jaffer employs several techniques depending on your specific anatomy and needs:
Liposuction: Used when gynecomastia is primarily caused by excess fatty tissue. Small incisions are made, and fat is removed using specialized cannulas.
Excision technique: Necessary when glandular tissue or excess skin must be removed. An incision is typically made around the areola border to remove tissue while minimizing visible scarring.
Combined approach: Often the most effective method, combining liposuction for fat removal with excision for glandular tissue removal.
Extended excision: For severe cases with significant skin excess, additional incisions may be necessary to remove excess skin and achieve optimal contours.
How long does gynecomastia surgery take?
The duration of surgery varies depending on the complexity of your case and the techniques required. Simple liposuction procedures typically take 60-90 minutes, while more complex cases involving significant tissue excision and skin removal may take 2-3 hours. Dr. Jaffer will provide you with a specific time estimate during your consultation based on your individual treatment plan.
